The game takes place in a futuristic city called New Himuka, where a psionic hormone was discovered in the human brain that granted its citizens extra-sensory powers. The story follows a young man named Yuito Sumeragi who must uncover the mysteries of New Himuka.

Yuito has powers of psychokinesis as he fights plant-like monsters called Others. Others are mutants that fell from the sky for unknown reasons, bringing with them a hunger for human brains. They are immune to conventional weapons and require the use of these new psionic abilities to dispatch them.

He is joined by the Other Suppression Force, a team made up of other elite psionic warriors. The trailer shows Yuito performing some of his special psychic abilities to throw cars at enemies, manipulate steel beams, and even bring the foes closer to himself. Its gameplay looks highly reminiscent of games such as Devil May Cry, NieR: Automata, and Astral Chain.

Scarlet Nexus has also been confirmed for Smart Delivery, ensuring that players will be playing the best version of the game whether they are playing on Xbox One or Xbox Series X.
